If you wish to refer a case for Manipulation or Hydrotherapy at Brentknoll Veterinary Centre, please complete the referral form below.

If you are a pet owner, please contact the practice either by phone or using our main contact form. Alternatively ask your vet to complete this form for you, please note this form is for referrals and is therefore for vet use only.

Requests shall be dealt with directly by the rehabilitation team, please note that Sharon and Paula are in practice on Tuesdays, Wednesdays, Thursdays and Fridays so please allow for a short delay for your reply. If your request is urgent please call us on 01905 355938.
